In our hospitals, patient safety is part of the organizational culture. A management approach that is far from punitive and accusatory approaches, supports open communication and encourages employees has been adopted. An environment of mutual trust has been created for all our employees to talk freely about safety issues and how to solve them. A systematic program is implemented to sustain and continuously improve the patient safety culture in our hospitals. In this context, trainings are organized to raise awareness on patient safety, surveys are conducted to measure employees' perception of safety culture, and practices are carried out to encourage employees to report errors. Reported errors are first comprehensively analyzed, root causes are examined and improvements are made to prevent recurrence.
